1Age & Citizenship

Age 21-38

This age range offers the best outcomes for healthy pregnancies based on medical research.

U.S. Citizen

Required for legal protections and contract validity.

2Pregnancy History

At least one full-term pregnancy (37+ weeks)

Proven pregnancy history demonstrates your body can successfully carry to term.

Currently raising at least one of your own children

ASRM guidelines require surrogates to be actively raising their own child to ensure emotional readiness and stability throughout the journey.

No more than two C-sections

Additional C-sections increase medical risks for both surrogate and baby.

No major pregnancy complications

History of preeclampsia, gestational diabetes requiring medication, or other serious complications may disqualify.

3Health & Lifestyle

BMI under 30

A healthy BMI reduces pregnancy risks and improves IVF success rates.

Willing to undergo IVF injections

The IVF process requires subcutaneous and intramuscular injections to prepare your body for embryo transfer. This is a standard part of every surrogacy journey.

Willing to complete psychological evaluation

All surrogates must pass a psychological consultation with a licensed mental health professional to ensure emotional readiness.

Non-smoker (including vaping and marijuana)

No tobacco, vaping, or marijuana use of any kind. We test for hidden nicotine markers.

No secondhand smoke exposure

Your household must be smoke-free.

No anxiety or depression medications

Current psychiatric medications may affect pregnancy safety.

No recreational drug use

We conduct comprehensive drug screening throughout the journey.

4Living Situation

Live in a surrogate-friendly state

California, Texas, Florida, Nevada, or Oregon. These states offer the best legal protections.

Stable housing

You need a safe, stable environment throughout your pregnancy.

Reliable transportation

You must be able to attend all medical appointments.

5Financial & Legal

Financially independent

Not receiving government assistance (TANF, SNAP, Section 8, Medicaid).

No felony convictions

Background check required for all applicants.

Spouse or partner must pass background check

If married or living with a partner, they must also undergo and pass a background check.

Valid government-issued ID

Required for legal contracts and medical records.

Why We Accept Only 1 in 60 Applicants

Most agencies will accept anyone who meets basic criteria. We do not. Our founder, Samantha Giermek, was a surrogate herself in 2017 — she knows firsthand what it takes to have a healthy, successful journey.

Our screening process includes a third-party IVF nurse review of your complete medical history, OB/GYN records for ALL pregnancies, and drug testing that catches hidden nicotine markers most agencies miss.

This selectivity is not about exclusion — it is about protection. We protect surrogates from medical complications, intended parents from heartbreak, and babies from preventable risks.

What Would Disqualify Me?

Transparency matters. Here are factors that would prevent you from becoming a surrogate with us.

Currently pregnant or breastfeeding
History of preeclampsia, eclampsia, or HELLP syndrome
More than two cesarean sections
History of postpartum depression requiring hospitalization
Current use of anti-anxiety or antidepressant medications
Receiving government assistance (TANF, SNAP, Section 8, Medicaid)
BMI over 30
Any tobacco, vaping, or marijuana use
Living with someone who smokes indoors
Felony conviction on record
IUD or Nexplanon (must be removed before application)
